With the game tied at 2, Pagán entered to close things out. But as he followed through on his first delivery to Cubs' Nico Hoerner, he immediately hopped off the mound in pain and crumpled to the ground. Reds training and medical staff rushed to his side as he lay on the grass. When helped to his feet, Pagán couldn't put weight on his left leg and was assisted into a cart—a sight no baseball fan wants to see.

Reds reliever Jose Franco stepped in, walking Hoerner but managing to escape the inning without damage, sending the game into extra innings where the Cubs ultimately prevailed.

This injury is the latest setback for Pagán, who has six saves this season but has struggled with consistency. Entering Tuesday, he carried a 6.43 ERA with three blown saves across 14 innings. The previous night, he couldn't protect a one-run lead, surrendering a game-winning solo homer to Michael Conforto in a 5-4 Cubs win.

Interestingly, Pagán had a similar scare last month after the final pitch of a 2-1 victory over the San Francisco Giants, though he downplayed the severity the next day. This time, the injury looked far more serious, leaving the Reds' bullpen in a precarious spot as they await further updates.
